1. Application Form (Form XIV)

• This is the standard copyright application form.

• Filled separately for each work category (literary, artistic, musical, etc.).

2. Details of the Work

• Title of the work

• Language

• Author’s name

• Year and country of first publication (if applicable)

3. Declaration Form (Form-XIV Statement)

• A signed declaration stating originality and ownership.

4. Copy of the Work

• Submit 2 copies (physical or digital) of the actual work being copyrighted.

5. NOC (No Objection Certificate):
Depending on the type of work:

• From publisher, if published by someone else

• From co-authors or other creators, if more than one person involved

• From person/agency if a photograph/image/clip/logo etc. is used in the work

6. Power of Attorney (if filed through an agent)
7. ID Proof of Applicant

• Aadhaar, PAN, Passport, or any government-issued ID.

• Email ID and Mobile number

• GST number (if applicable for business-related works)

• Company registration certificate (if copyright is in company’s name)
